And confirm who the provider actually is: as written, the provider is
Michael Schultz as a private person, using the GmbH's address.
That is fine and needs nothing further. If the GmbH is the
provider, § 5 additionally requires the Handelsregister and
Registernummer, and the Vertretungsberechtigter (Geschäftsführer) — and
the heading should name the company, not you.
